Travel History
  • List of previous international travel (with entry/exit dates on a separate page)
  • Details of any visa refusals (with refusal letters or reasons)
Proof of Assets
  • Property ownership documents
If Sponsored
  • Original Letter of Invitation signed by the sponsor in Poland
  • Sponsor’s Passport Copy (photo page + valid residence permit if the sponsor is a non-Polish citizen)
  • Sponsor’s National Identity Document (Polish ID card – Dowód osobisty – if Polish citizen)
  • Sponsor’s Residence Proof (Certificate of Residence from the local municipality or recent utility bill)
  • Rental Contract or Property Ownership Documents
  • Employment Proof: Work Contract (Umowa o pracę), Last 3 Salary Slips (Odcinki wypłaty)                  Or Business Proof (if self-employed): Company Registration (from CEIDG or KRS), Recent Tax Return (Deklaracja podatkowa)
  • Sponsor’s Bank Statements (last 3 to 6 months)
  • Relationship Proof (e.g., Family Registration Certificate from Pakistan with certified English or Polish translation if required)

Note: Without a formal Letter of Invitation, preferably officially registered and notarized in Poland (Zaproszenie registered with the Voivodeship Office – Urząd Wojewódzki), sponsorship may not be considered valid. Sponsorship does not eliminate the need for the applicant to submit their own financial documents — both sponsor and applicant documents are required. The Polish embassy may also request additional documents such as the sponsor’s tax clearance, housing verification, or travel history.
